Haemorrhoids (Piles)

Overview Commonly known as piles, haemorrhoids are abnormally engorged and swollen blood vessels in the anus and lower rectum. These may rupture and cause bleeding or slip out of place during defecation. Piles cause discomfort and itching, affecting a person's daily lifestyle. This is when treatment may be required, and is referred to as haemorrhoidal disease. It is a common condition, but many people are too embarrassed to see their doctor about it until the pain or discomfort becomes unbearable, or when there is bleeding, causing worry that it may be something more serious.

There is more than one type of haemorrhoid. Internal haemorrhoids develop within the rectum. They may be present but cause no pain or discomfort. However, straining during bowel movement may cause them to bleed and protrude as a result of prolapse. Defecation can cause trauma and bleeding, while the presence of stool and constant moisture in the anal canal can lead to anal itchiness, although itchiness is not a common symptom of haemorrhoids. Less common - the haemorrhoid protrudes from the anus and cannot be pushed back inside, a condition referred to as incarceration of the haemorrhoid.

External haemorrhoids develop around the anus and can be felt as bulges at the anus, but usually cause few of the symptoms typical of internal haemorrhoids. When irritated, they may itch or bleed. External haemorrhoids can cause a very painful anal lump when blood clots inside them (thrombosis).

Causes

There are multiple factors resulting in a person having symptomatic piles. These are generally associated with any condition that can cause an increase in intra-abdominal pressure, including:

Persistent straining during bowel movement and chronic constipation or diarrhoea, or even prolonged sitting on the toilet. The extra pressure during prolonged straining causes these vessels to swell and enlarge.

Inadequate fibre intake

Obestiy

Gentics. The tendency to develop haemorrhoids may run in the family.

Tumour. A less common factor is the existence of an intra-adbomonial or pelvic tumour.

Pregnancy. A common cause for piles for women is during pregnancy, where the growing foetus and labour exertion causes pressure on these blood vessels.

Symptoms

The symptoms of piles can mimic and mask the symptoms of cancer. It is important for any patient complaining of symptoms that might be attributed to piles to consult their specialist surgeon for a full examination, before dismissing their symptoms as mild and haemorrhoid related.
Some signs and symptoms that patients can look out for include:
  • Bleeding at times other than defecation
  • Sensation of incomplete emptying that is present all the time, despite defecation
  • Change in the calibre and character of the stools
  • Change in bowel habits
  • Obvious hard mass at the anus
  • Unexplained loss of appetite and weight

Treatment

The treatment of piles depends on the severity and the type of symptoms affecting the patient. Simple preventive measures include having a balanced diet and adequate water intake to allow regular bowel movement without straining. If this doesn't help, other treatments available include:
  • Oral medications or topical suppositories. These can be used to treat small bleeding piles.
  • Ligation. This rubber band treatment works effectively on internal haemorrhoids that protrude with bowel movements. A small rubber band is placed over the haemorrhoid, cutting off its blood supply. The haemorrhoid and the band fall off in a few days and the wound usually heals in a week or two. This procedure sometimes produces mild discomfort and bleeding.
  • Injection. For this procedure, the doctor injects a chemical solution into the haemorrhoid, causing it to shrivel and subsequently fall off. This method is relatively painless and can also be used on bleeding haemorrhoids that don't protrude.
Piles that are not prolapsed can be ligated or injected. These are simple procedures that can be done in an outpatient clinic and do not require hospitalisation. But these procedures may not be extremely effective, and may require more than one treatment.

More definitive measures are needed for severe cases, such as when clots repeatedly form in external haemorrhoids, or Iigation fails to treat internal haemorrhoids, or when a protruding haemorrhoid cannot be reduced, or when there is persistent bleeding. Surgical techniques under general anaesthesia include:
Haemorrhoidectomy:
This is surgery to remove excessive tissue causing the bleeding and protrusion. It is the best method for the permanent removal of large, prolapsed haemorrhoids.

Haemorrhoidectomy can be performed in the conventional manner, or through stapled haemorrhoidectomy. This is a form of surgery performed under general anaesthesia. A device is used to excise the piles internally and the internal wound is closed via a row of "staples". Compared to the conventional "open" method of haemorrhoidectomy, where there is a raw wound, stapled haemorrhoidectomy causes less discomfort and a shorter duration of pain to the patient in the immediate postoperative period. It's effective in treating large piles that protrude from the anus during defecation.

Transanal haemorrhoidal dearterialisation (THD):
This procedure involves using an ultrasound to locate the problematic blood vessels and stitching the piles.
As methods may vary slightly from person to person, patients are advised to consult their surgeons to figure out the right treatment for them.

Permanent Free Pile locations

You're riding down the street, just minding your own business, when suddenly you see that thing you've been wanting sticking out of a box of stuff on the curb. You stop and take a closer look, and Behold! On the side of the box is scrawled the word "FREE".
Free piles are a special bit of Davis magic. People put their unwanted belongings, such as clothing, books, kitchen gadgets, food, furniture, or anything else, out on the curb, and they conveniently disappear in an environmentally and socially responsible manner. Free piles are not just one-time items or even piles of stuff left near the sidewalk. They are year-round depositories/exchange areas for people to leave and take no-longer wanted items.
Items typically are disposed of if they aren't taken after 7 days.
Transient Free piles frequently occur after yard sales and during move-out time. It never hurts to place a paper sign marked "FREE" on your item, just so it is clear to everyone that it is OK to take.
Free piles can be found throughout residential areas of Davis, they are more common in some areas than others. They are particularly found in older neighborhoods. Sometimes free piles are advertised on Craigslist or Freecycle (though this is against the Freecycle rules). Other times they're unadvertised.
In apartment complexes, the area near the dumpsters frequently ends up being a de facto free pile for items that are still useful.

Free Pile Etiquette

  • Use caution when taking items that are not specifically marked as free. If in doubt, ask first.
  • Conversely, free pile creators should label piles as free if possible, so as not to contribute to confusion.
  • Take what you will use, leave what you don't for someone who will.
  • Leave the free pile at least as neat as you found it.
  • Don't make noise if it's after dark; neighbors may be sleeping.
  • Free pile creators should find alternate disposal for items not taken within a reasonable period of time.
  • Avoid leaving items such as books when it's about to rain - people are unlikely to sort through a pile of sopping wet stuff.
  • Large items such as mattresses often cause more trouble than they help others. The rules of piles vary.

piles during pregnancyPiles disease during or after pregnancy is common and painful side effect of pregnancy. The load carried by the pregnant women results in aggravating the piles. The standing, laying down or even sitting can become uncomfortable due to burning and itching sensations associated with piles. Mostly during the third trimester of a woman’s pregnancy piles diseases appears when the increased pressure on the rectum and perineum are the most significant.  The pressure on the anal area during the painful and long process of childbirth can also lead to piles disease.
Lets first focus on piles during pregnancy – According to different studies, approximately one-third of all pregnant women suffer from hemorrhoids at some point during or immediately following their pregnancy, even if they have never suffer from them at the past. The main causes of piles disease during pregnancy are lots of straining during bowel movements creating pressure on the veins or pressure created by the fetus in the womb during pregnancy and chronic constipation. The severe piles can result the inflamed blood vessels to burst resulting in bleeding.
Generally during pregnancy the growing weight of the fetus and its increasing weight in the womb create a strain on the anal veins. In general the blood circulation in the body of a woman during this time is already very high which results in the languid movement of blood in the veins leading to piles disease. Constipation is also very common in pregnant women. It is also one of the main causes of piles among women. The lot of pressure to be applied during the bowel movement also creates pressure in rectal veins which swell resulting in piles disease. Third and the last main cause is the birth process itself which requires great pressure to be put in by the women’s body leading to the occurrence of the disease.
The main problem with piles disease while pregnant is that it’s impossible to go for medications for treating it during this crucial stage to avoid any danger to the fetus. If necessary then very less invasive methods will be adopted to cure it like usage of creams, ointments and suppositories.
Though it is difficult for a pregnant woman to get a treatment done during this stage yet to relieve her of the discomfort and irritation few treatments can be used. If constipation is one of the causes of piles disease then you should consume more fibre rich food, drink a lot of water and if required she can also take stool softener. You can also eat figs for better digestion.
In case the pressure and the increasing weight of the baby is the cause of piles disease then than you should try to ease pressure on your abdomen. For doing this, you should lie on your left side or lift up your feet in the air for 20 minutes every few hours. This will not only result in alleviating the pressure on your abdomen but also reduce the amount of swellings on your hands and feet that pregnant women suffer from.
Piles disease can also be treated by hot water bath in saline water. Soaking in warm water and washing the infected area with plain water for about 15 to 20 minutes at least once a day will give instant relieve and also result in reduction of itching, pain and stress which accompany piles disease.
As surgeries cannot be taken up as a treatment of piles disease for a pregnant woman but as it creates a lot of pain and discomfort to the patient therefore, it shall be cured. to relieve the uncomfortable situation treat yourself with diet, exercise and other relieving techniques. Holistic methods can be used as well as some medicated creams and ointments can also be prescribed by the doctor.
If the treatment can be delayed till the baby is born then the women can opt for simpler techniques. But for the treatment of piles disease both during pregnancy and after child birth have to include the changes in diet by consuming more fiber rich food and figs, taking hot bath in saline water, stool softeners that will ease up the bowel movement through rectal veins, increasing the amount of water drinking, taking in of antidotes to relieve the pain caused by the piles disease and keeping hygienic toilet habits. These will help the woman to relieve her of the pain and the discomfort caused by the malignant disease.
After the childbirth, piles disease can be treated just like in normal condition. click here for more info on piles disease treatment.

piles disease treatmentPiles disease is a common disease nowadays. The number of people suffering from piles is huge.  The changing life style has infused so many diseases in our bodies. The dietary habits, the processed foods and unhealthy living styles are the main cause of piles.
Piles disease is neither a disease nor an infection. Therefore, it is not curable by any of the conventional methods. Though the conventional medicines/surgeries can relieve the patient of some of the piles symptoms it will not be cured if you won’t change your lifestyle daily habits. So, natural treatments are its best remedy.
Piles disease is mainly an outcome of the swelling of the rectal veins around the anal. A clump is formed inside or outside the rectum. There are three spongy pads in the anal regions. Even if one of the pads slips downwards leading to the filling up of the blood vessels in the cushion which is the cause it to swell and it results in piles. Piles can be internal or external or a patient can suffer from both types at the same time. Piles are also called hemorrhoids.
The causes of piles disease are straining during bowel movements, chronic constipation and pregnancy. Obesity may also result in piles because of the pressure imposed by our body on the rectal veins. As well as the sedentary jobs may also contribute to the occurrence of the disease. Heavy weight lifting also leads to piles. People who have loose motions frequently are also prone to piles because of the strain imposed on the lower part. Piles may also be a genital disease which is transferred from generation to generation. It is mainly found in adults. Thus, the treatment of piles must be initiated as the first piles symptom is seen. This will help in reduction of the discomfort that will be accompanied by the disease.

Rubber band ligation is a procedure to treat internal hemorrhoid with a special applicator. In this a small rubber band is placed at the base of the anal. The rubber band cuts off the blood supply to the internal hemorrhoid and thereby, the hemorrhoid fall off in four to five days.
Infrared coagulation is another method of treating hemorrhoid. It uses an infrared light source which treats the infected area. This method will cause the shrinkage of the dilated veins as blood does not flow through the area.
Injection sclerotherapy is a technique of injecting chemical into the infected area. This irritating chemical leads to inflammation and closure of the veins leading to shrinking of hemorrhoid.
In case of large hemorrhoids, a surgery can be taken up resulting in the removal of the hemorrhoid.

What is Piles Disease ?

Piles disease is also called hemorrhoids. It is an uncomfortable condition that may affect us and our surroundings. The lining of the anal canal contains three spongy pads of tissue which are like a seal for the anal canal to remain closed till lavatory passes through it. When one of these soft pads slips downward because the tissue around cannot hold it, then the small blood vessels within the cushion fill up with blood resulting in the swelling up of the cushion. Thus, piles disease is a condition in which the veins around the anal swell up. There is a clump formed inside or outside the rectum.
The main cause of the piles disease can be straining during bowel movements, pregnancy and chronic constipation. But if you are constantly sitting at one place or you are too corpulent resulting in straining of your rectal veins, than it may also result in occurrence of piles disease.
Though piles disease develops from inside the anal canal yet they may hang out on the outside of the rectum. Therefore, doctors compile piles disease into mainly three types.
Firstly, there are the first-degree piles. They always remain inside the anal canal and are swollen tissues. Usually they are painless but on some cases they might cause some pain as well.
Secondly, there are the second-degree piles. They prolapsed when faeces are passed and return to their normal position.
Thirdly, there are the third-degree piles and the most painful ones also. They prolapsed or are pushed when faeces pass or they may come down themselves but they do not return to their normal position after the faeces are passed through them.
Age is not a barrier for the occurrence of the Piles disease but it is more common among adults. Men and women both are prone to this disease. In fact, four to twenty five out of hundred people suffer from this disease.
Usually, piles disease is a temporary problem. Experts believe that they are caused by continuous high pressure in the veins of the body when a person stands upright. It may also occur during pregnancy among women because of the hormonal changes and pressure from the baby. Straining during the bowel movements may also result in piles disease. This may be caused by lack of fibres in our diet or lifting heavy weights.
Piles disease has mainly the following symptoms. Usually  the main symptom is bleeding, with bright red blood. Secondly, itching and irritation around the anus and there may be swelling around the anus. Also, a lump may protrude on your anus and a slimy discharge of mucus can be noticed which is mainly the cause of itching and irritation. Last but not least, there may arise a pain after a bowel discharge in case you have an external pile disease.
In order to cure the piles disease, the patient should try to bathe with warm water. This will reduce the irritation and he shall also keep the anal area clean. Straining during the bowel movements shall be avoided. He may also take some medicines such as ispaghula husk (e.g. Fybogel) or mild laxatives such as lactulose, which will soften your faeces or apply smoothening creams, ointments and suppositories to reduce pain and irritation. The patient may also undergo non-surgical treatments such as banding and sclerotherapy. He may also take surgeries to heal the lump in the anus to reduce pain.
Thus, piles disease can be treated dif there is a permanent problem causing strained bowel movement, pain, irritation and itching. It should be undertaken after consulting with a doctor.
